Those who will be attending 2012 Mid-America PETS should forward their
Registration Forms to individual District Registrars on or before February 17, 2012 to receive the early registration discount.

This includes Club Presidents-Elect and Nominees, Assistant Governors, Guests, Facilitators, etc.

Each Rotary District has its own registration form.  The PETS registration form is submitted to your district;  the hotel registration form is submitted directly to the Holiday Inn City Center. Hotel reservation deadline is February 21, 2012.


Mid-America PETS Registration Guide

All participants will register with their respective Districts.

Each District will designate a Registration Coordinator, who will communicate directly with the MAPETS Registration Chair.

Each person who attends MAPETS, including guests and speakers, must complete a registration form.

Each District’s registration form will be posted to the MAPETS website.

The payment for each District will be sent to the MAPETS Registration Chair, who will forward to the MAPETS Treasurer each payment, along with a summary sheet that breaks down the payment: number of full registrations, number of meals only registrations, number of individual meals.

The full registration fee is payable to your district.  The meals only registration fee is available for guests.  Individual meal registration is available for guests.

Persons paying the full registration fee will receive a binder containing all of the MAPETS materials.  Persons paying the meals only registration fee will not receive the binder and the MAPETS materials.

The initial registration period will end on February 17, 2012.  Late registrations, which will include a $25.00 late fee, will be accepted until March 4, 2012.  Following that date registrations will be accepted only at the event and will include the $25.00 late fee.

These forms are writeable .pdf formated documents.  The free Acrobat Reader is required to open these documents.  You can download the free version of Adobe Acrobat to view the documents, complete them and print to be mailed or faxed by clicking the "Get Acrobat Reader" button.  To save the completed document on your computer, you must have the full version of Adobe Acrobat.
